Ice Skating Fundraiser

1) I understand that ice skating involves inherent risks of injury. I voluntarily assume all risks associated with participating in the Covenant Resources Miscarriage Ministry Inc. Ice Skating Fundraiser to be held at Hamilton Community Center and Ice Arena. I voluntarily assume such risks for myself and for each person whose name is listed below as a part of my party.

2) I hereby release, waive, and hold harmless Covenant Resources Miscarriage Ministry Inc., its organizers, volunteers, and affiliates, from any and all liability, claims, or demands for injury, illness, or damages arising from or related to my (or my child’s) participation in this event.

3) If I am registering a minor (under age 18), I certify that I am the parent or legal guardian of the minor participant and give my permission for them to participate. I accept full responsibility for their participation and agree to this waiver on their behalf.

4) I certify that I (or my child) am physically able to participate and will follow all posted and verbal safety instructions.
